Seems to happen when clicking on an image file, anywhere, and using the spacebar to preview quickly - I see a red rectangle appear and then go away usually. It seems to happen periodically. I've read it could be a faulty graphics card. Is this true? It's a 15 in. MBP 2018 and a Radeon Pro 555x card.and Intel UHD Graphics 630. Has anyone else seen this too?

You are saying this is a periodic temporary glitch using quicklook in Preview only...(?)
Verify you Uninstall all third party apps that are Cleaners/Optimizers/VPN/Anti-Virus
Try a SafeBoot https://support.apple.com/en-us/HT201262
Takes noticeable longer to get to the login screen, does a 5 minute disk repair before it fully boots up, and certain system caches get cleared and rebuilt, including dynamic loader cache, etc.
You can Login and test, Reboot as normal and test . Caches will get rebuilt automatically.
If no resolve—
For one off anomalies like yours, reinstalling the macOS on top of your existing macOS may be the way forward.
