What you are seeing is perfectly normal and expected behavior. When you apply that small update, it will not update the OS version on your Mac. The update is necessary however, if you want to use iTunes with your iOS devices on the Mac.
I downloaded the update and applied it quite some time ago and It did nothing more than let my iPhone and iPads sync and backup with my Mc.

I’m not sure what it is exactly that you are asking, but if you are asking if you can backup and sync an iOS Device running iOS 14:with a Mac that is running El Capitan, 10.11.6, the answer is yes. I am running 10.11.6 on my MBP and I sync and backup my devices that are all running iOS 14.7.1.
Sorry my question is not clear. When I connect my iPhone to an iMac (running Mojave OS 10.14), I get a window saying

But I do not want to update my iMac OS to Catalina.
In fact, my iMac does seem to recognize my iPhone. What's going on?

If so, and they didn't help, I suggest that you contact Apple Support directly. To do so, simply select the "Get Support" link in the upper right-hand side of this forum's web page.
If after following the directions in that article, are you saying that it is recommending that you update your Mac to macOS Catalina?
