How do I reactivate my account on iTunes and App Store?

If you mean your account has been disabled, contact Apple Support as stated in here: If you see a message that says “Your account has been disabled in the App Store and iTunes” – Apple Support (AU)
If you mean it is inactive because it has not been used in a long time, just log in. If you have forgotten your password, go to appleid.apple.com and click "Forgot..."
