How can I request a refund for unauthorized purchases months prior?

I have a number of charges for apps and unauthorized downloads and would like a refund please (arcade, epix and more)

If you got an email receipt, use the 'Report a Problem' link in it. Otherwise, click here and ask the iTunes Store staff for a refund.
