How do I back up
all apps? Especially those apps no longer available in the App Store?
If you do an iCloud back up this will back up all of the apps stored on your phone.

Clarification: I set up my new phone from an iCloud backup - apps that are no longer in the App store did not fully load and now no longer work.
Should have loaded my new phone from old phone not from the iCloud. iCloud was 'faster' but I lost apps.
Perhaps the apps you can't find any longer on AppStore weren't updated by their developer to cohabit with the latest IOS version. Contact their developer teams and ask them.
The same thing would have happened if you had created a backup using a computer with iTunes (PC) or Finder (Mac).
Yes - true. Issues occurred when I purchased a new iPhone and setup the new phone using the iCloud backup. What I should have done was transfer my information phone to phone and not use iCloud. The 'missing' apps are still installed on my iPad and still work - so it's an issue of the apps not having been backed up to iCloud and their no longer being in the App store.
Bottom line, still need to figure out how to make sure all apps are fully backed up to iCloud without relying on the App store.
Thanks - I did. But it didn't recognize the previous purchase - app loads but says I need to pay for premium even though I already purchase premium.
