If it is getting HOT, not just mildly warm, then I would worry about damage, and suggest replacing the power brick and integrated cable. Generally not cheap.
At the minimum, do not leave it unattended while you are charging the Mac.
If cable manages to melt the insulation inside the white sheathing, it could start a fire.
You might also consider taking it to an Apple Store and demonstrating the heat generated to the Genius Bar.
That basically means you are pushing more current through the wire, then it was rated for.
Is this cable integrated into the charger brick, or is it a separate cable from the charging brick?
If integrated to the charge, is it an Apple manufactured charger, or a 3rd party charger?
