Mac Facetime calls fail, apparently related to "avconferenced"

On launching Facetime I got a message asking if I wanted something called avconferenced to run. Not knowing what it was, I said No. Now Facetime calls fail, outgoing and incoming.

Is there a way to get avconferenced back?

In looking further into this I see that Facetime no longer works on any devices on our wifi network (which is working fine). There's a brief connection for just an instant and then the message "Call failed."

All I can think is that something may have happened when the phone company switched us to fibre a few days ago but we still use the same Airport Express router.
