Ok FINALLY figured this out - as far as I can tell My Mercy does NOT have a QR code anywhere that I found - BUT - SSM My Chart does. My My Chart account didn't have my vaccine listed in it, but when you go to the Covid 19 page - there is a button to "update" your vaccine record from the state database. Hit that, it'll then load the vaccine info and THEN you'll be able to get the QR code you were all talking about. Thanks all for your help. I did message Mercy though to ask them if/when they are going to support this because they are a big healthcare system and they really should. We'll see if I get a response.

Does your health provider support this? Did they send you a code to scan? Reread the related article in the upper left corner.
Add verifiable COVID-19 vaccination information to Apple Wallet and Health - Apple Support
You can download verifiable vaccination and test result records if your vaccine or test result provider supports this format. Check with your provider to see if they support verifiable health records.

My provider supports the format and adding my vaccination card went flawlessly.
Success. I was able to chase this down. If you go to this CDC site and then to your states website, (in my case for California), I was able to enter my information and get a verified link that I was able to get my verified vaccination QR code. I was then able to scan and add the verified record to both health app and wallet.

If nothing else is working for you and this doesn't help, maybe you should contact your provide and make sure they support this.
Without showing too much personal info, when I go into my medical records on my provider’s patient web portal, I can select immunizations from the menu in the portal. When I do that, it brings up this dialog box stating that I have had 2 of 3 Covid vaccinations.
if I tap on QR codes, it takes me to a screen where there is a QR code that I can scan from the health app. When I did that, it brought up this screen. Then I was able to tap on Add to Wallet and Health App.
The reason that the message in my first screenshot states that there was no update from state registries is because I searched for the record of my third shot. I actually received my booster, two months ago, but I got it from a local pharmacy rather than through my provider. I am immuno-compromised so I wanted to get my booster as soon as they were available.
I would contact your provider then. Because if the record in Health when you open that second dose info doesn’t say “verified” at the bottom then it won’t let you add to Wallet.
i tried my husbands first thru Mercy and it had this problem, but the same vaccine record thru SSM My Chart worked and was verified.
Can you access your medical records through an app or a web portal for your provider? I use MyChart and I was able to scan the code from the provider site under my Vaccination records.
It is not an Apple bug. It is a feature that is not supported by your medical provider.
Unfortunately My Mercy doesn’t have anything like that. Only a printable list of immunizations. And all that info transferred over into the Health App, dates, lot number, vaccine type etc. But no way to add to Wallet unless Apple fixes their bug. Several commenters have had the exact same problem so hopefully they fix it.
But what I am asking is can you go to your provider’s patient portal, find your Covid vaccination record and look for the QR code in there?
There is no QR code for anything in the My Mercy system. You just log in, and view your info. No QR codes. So I have no idea what that is in reference to. I don’t know anyone in our area with any kind of QR code when they got vaccinated.
I’m sorry but I don’t know what to suggest at this point other than to contact your provider for help.
My provider was one listed - and my records show in the vaccination section. But there is no link to "Add To Wallet" and it does not show up in the summary to Add to Wallet like the example photo shows. Guessing this is Apple's issue and a bug because many people are having this same issue.
Hi, thanks for being willing to help with this. I also have myChart for my provider and although I have both of my vaccine records both in myChart and in health app, there is no option for adding to wallet. On my record it says "Patient Reported: Yes" and I'm wondering if that possibly has something to do with the issue?
When I read the QR code, I get ‘add to health’ but not ‘add to wallet’ option. I have already added it to health app and pressing that again just shows a duplicate record message. My vaccine is in the health app, but I cannot add it to wallet. It will even display the QR in the app.
I am having exactly same problem as you described above Jackson Health populated all COVID info to my health app but there is now way to upload to my wallet ???
very frustrating after all apple keeps pushing this great achievement 🤬
No "Add to Wallet" Link???
