how can I add my CDC Vaccination Card to my iPhone?

When IOS 15.1 became public, one of the new features allows a User to place his/her CDC vaccination Record Card on their iPhone. Is there an Apple Document that outlines the steps to put the Vaccination Card on an iPhone?

You need a verifiable digital record provided to you by a health or government entity. Your vaccination card cannot be added to the wallet.
