Apple Health won’t accept my twins QR code Vaccination receipts as separate entries. I can upload one to Apple Health and Wallet and then when I go to add the other child it says the Data already exists (duplicate data).

Good luck, and Apple you should really fix this. Got a new phone and it didn't transfer over my vaccine record to wallet, even though the entry was in my health app (although hidden in a weird place). Also the state COVID vaccine doesn't show up under my immunizations, so took me a while to figure out where it was saved. Overall, great feature, love the vaccine card on my phone, but this was a bit of work.

Got it. Seems this was addressed in the newsest iOS version 15.3. Updated and they imported without issue.
Could be. I suppose a work around would be to add the cards, take screen shots and keep them in photos (just so they are all together). At least you can add them. We're a little more ... haphazard in the states.

They were issued by the Ontario Ministry of Health. Separate vaccination receipts for each child under their personal health card numbers. I’ve even deleted the first child’s record that was uploaded into Apple Health and uploaded the other child’s record first and it works so no issue with the individual records.
I was able to upload my husband’s record to my Apple Health and my wallet along with the one twins record. I’m thinking it’s an issue with it not recognizing the records that have the same vaccine dates or possibly birth date.
I have exactly the same problem with twins. Either record can be on a phone by itself. I’ve tried each record on a different phones and they are correct I’m health and wallet. Even the health app recognizes correctly initially. Both records are correct it’s just adding the second. It’s getting them on the same phone.
I'm wondering if you can't have more than one individual's record loaded (that was my first thought). I've been searching for an answer but haven't found one yet.
Hmmm, didn't work for me. When I delete the source, it deletes the data/card as well. Maybe its different because im in Canada? At any rate, still looking for a solution.
