Hey there! Welcome to the community.

The easiest way to clean out your earpiece on an iPhone is to get some sticky putty and press it into the speaker grill. When you pull the putty, it will pull out the dirt as well. This link from Apple Support should provide detailed instructions on how to clean your iPhone: https://support.apple.com/en-us/HT207123

Now, in regards of future cleaning, it may be worth taking it to an Apple Store or an Authorized Service provider to clean out the earpiece. While I cannot speak on behalf of other AASPs or Apple Stores, I know that my store does not charge for cleanings.

To answer your question on future reliability, it really depends on what you would consider normal use for your iPhone. Of course, with the puncture in the speaker mesh, that means dirt, especially with ferromagnetic materials, can enter the main speaker housing, which can cause issues in the long run.

Solution? Keep the phone clean.

Hope this helped!
