As to why the phone icon disappears, it’s because the connection to the phone was lost. But why it was lost will take some troubleshooting on your part. Make sure you have the latest version of iTunes (https://itunes.com), and see if your antivirus is blocking it for a start. Does it disconnect when you unplug the cable, or does it disconnect when still plugged in? Do you have Connect over Wi-Fi enabled?
