NFC Scan issues

Having issues reading NFC Tags on iPhone 12 Pro. I'm using NTAG215 round disks with iOS 15.1.1 and can't seem to scan. I have some disks setup in shortcuts to trigger home automation but it is hit or miss whether the phone scans them. I use a third party app called NFC Tools and it detects/scans every time. Is there an issue with iPhone or iOS that anyone else is experiencing?

Nfc reader I dont have any nfc reader in the control centre on my i phone 13. So what should be done to get it. Or i shud get a third party software for that or it ll be fixed on next update

NFC TAG READER I have an 11 pro max with ios 15.0.2 and I can't find the NFC TAG READER in the control center. What should I do to find it?
