Transfer Apple card to new Apple ID and phone
What if I no longer have the phone or Apple ID that the Apple card is linked to? How do I transfer it to my new Apple ID and new iphone?

You cannot. You have to call Goldman Sachs at 1.877.255.5923 and cancel the card and then apply with the new Apple ID. You do not need a new Apple ID when you buy a new device. Your Apple ID is designed to be permanent.
