How do I purchase a new Apple watch as a gift and get it set up on the recipient's account?

You just purchase the Watch, the user would be responsible for activation with their Apple ID, which makes it theirs. If you wish to purchase a GPS+Cellular device, they will have to setup the cellular on the phone if they wish to use it. It is an extra charge on their cellular account, and you would not have access to do that on their cellular account. It can be done at anytime after they activate the Watch.
