Apple Wallet Vaccine Card showing middle name instead of last name

I'm not sure what topic this belongs under, but I've added my vaccination card to my wallet using the QR code method, but it shows my first and middle name. Not my first and last name. Any ideas about how to fix it? I've checked the health app and there it shows my first, middle, and last name.

When updating my vaccination information in health/wallet, new record is now "Unable to Add to Wallet" I've received my fifth COVID-19 dose, all through the same provider (Walmart). Until the first four doses, all was OK adding to health and Wallet. Now with the most recent dose, I scan the QR code which is available through Walmart pharmacy, and when doing so I receive the message "Unable to Add to Wallet". The vaccination record has been added to immunizations in Health. The vaccination card could not be added to Wallet at this time. When going into health, it shows the Source as www.walmart.com, and then "Not Verified - Unable to Verify" Any idea what may be happening? How can I solve this and have an updated vaccination card? Thanks in advance for the help.

Can't update COVID-19 QR code in Apple Wallet I used CVS to get my COVID-19 vaccines. After the first 2 doses, I was able to add it to Apple Wallet using the QR code from CVS website. I then had my 3rd dose and CVS generated a another QR code. When I tried to use iPhone (running iOS 15.4).1 camera to scan the updated QR code, I have the option to "Add to Wallet and Health". I selected that option. However, I kept getting the message "Unable to add to Wallet. The vaccination record has been added to the immunizations in health. The vaccination card could not be added to Wallet at this time". Also, Apple health app says it's "not verified." I tried to removed the old COVID-19 card (2 doses) in the Wallet app but still can't add the new card showing the 3 doses.. My Wife is having the same problem with the Walmart QR code, same as me. Her iOS is 15.4.1 as well. I find it hard to believe all these.QR codes are incorrect . Any ideas would be appreciated. Thank You !

Have an immunization record from healthcare provider in Apple Health, can't create a Wallet card I have immunization records for the covid vaccine in the MyChart app. I have Apple Health permissioned to import data from the healthcare providers including vaccinations. I see vaccination events in Apple Health Summary and under Browse > Health Records > Immunizations. But nowhere on either of those Health App immunization events in Summary or under Browse, can I find a way to turn those events into a card for the Wallet. What am I doing wrong? There were all sorts of news articles about how IOS 15.1 Health and Wallet would support adding proof of vaccination to the Wallet app.

This was fixed for me after updating my iPhone to iOS 15.2. Thank you Apple iOS team!
