If your father would have difficulty using Apple Watch on his wrist, it would not be a good device for him to own. I would seek other options for fall detection, which would be a better option for him. There are multiple devices designed to be worn around one's neck.
No. for Fall detection to be activated, the watch needs to detect the wrist. If its hanging around your neck it will assume its not being worn, so no fall detection.
