Considering an Apple Watch for the emergency fall detection but we spend lots of time in areas without cell service. Waste iof money?

Apple Watch without cell service. I had a heart attack in July 2021. Active guy, came out of nowhere.

i know these watches are not heart detection devices but we spend a lot of time in areas with either limited or no cell service.

Does the 911 still work or am I just wasting my money?
