The Apple TV app would be integral part of iOS 12.3 or later. You already have it, if you have an iPhone 11 (=iOS 13 or later). If it was deleted, then you can re-download it from the on-device App Store.

What is your country or region? In case that might be part of the issue. Accounts for some countries or regions do not get support for Apple video services. Availability of Apple Media Services - Apple Support
