How accurate is the location of the watch when the person/phone was over 10 miles away in a hospital?

Distance doesn’t matter; they could be 10,000 miles away. The accuracy depends on location of the Wi-Fi network the phone is connected to if indoors, or satellite access if very near a window. Find My knows the locations of most Wi-Fi hotspots around the world. In a hospital it may not be closer than “somewhere” in the hospital, unless the hospital has provided a map of Wi-Fi addresses of its individual routers. Many do, so their staff can pinpoint their location.
