I get a prohibitory symbol at start up. Tried pressing both "Cmd" and "R" button, but does not restart. Symbol returns.

How do I restart when the prohibitory symbol remains? I get a prohibitory symbol at start up. Tried pressing both "Cmd" and "R" button, but does not restart. Symbol returns.
