Clearing file folder with a question mark

I've tried all the startup keys, Hold shift, hold option, hold D, command option P-R and Command R. Nothing works. Any suggestions?

That means there's an issue with the hard drive and the computer cannot boot.


The only way to get rid of it, would be to fix or replace the hard drive.
