Can I suggest you contact Apple Support as you will be able to explain to them in more detail your requirements and they will be able to provide a better response.

You can also contact Apple Support as follows:

• Online:--> Genius Bar Reservation and Apple Support Options - Apple;
• Contacting a local Apple Store or AASP among the list provided above but also again as follows Apple – Location Selector; or
• Using the "Apple Support" app, available and downloadable from App Store.

A PUK code is generally associated with a SIM card so it is likely your iPad has or had a SIM card.

As such you will need to obtain your PUK code from your mobile service provider or the provider of the SIM card.
