Highlights in Preview are not always saved! When I highlight some text, press 'command' + 's' to save them and then close the pdf (also tried to 'export' the pdf), some of the highlights are gone. That happened with more than one document.
Below is the info for Mac, System and app:
Mac: M1
OS: Monterey v12.1
Preview: v11.0 (1033.3)
Any ideas whether this is a bug and/or how to fix this issue?

Does that behavior still persist if you boot your Mac into safe mode per the steps in this article? How to use safe mode on your Mac
It can take a few extra minutes to boot into safe mode. Your screen may flash or flicker while in safe mode. Those behaviors are expected. Testing safe mode can help isolate conflicts with startup items. Once you’ve tested that, you can restart to leave safe mode.
Should that behavior still persist while in safe mode, you’ll want to reach out to Apple Support so they can look into this further: Contact Apple Support
