How Can You Truly Silence an iPhone?

It's 3a, I can't sleep. I am in Silent Mode with the volume all the way down--phone partially hidden under the blanket so as not to wake my wife. I'm scrolling through news articles and my fingers inadvertently hits a embedded video and WHAM...volume 10 and I get slapped upside the head! AutoPlay is off, but being 1/2 asleep, I admit to starting the video. But, is there a reason it doesn't start playing without volume? Why does the video sound override the device settings? Can I stop it from happening, or do I need to plug headphones into my phone in preparation for my occasional insomnia?

I would appreciate any tips someone may have to avoid all sounds while in silent mode.

When you want to ensure your iPhone is silent, you can also then turn the media volume all the way down: Adjust the volume on iPhone

I did mention in my question that the Silent Mode is on, and the volume is all the way down...however, when the video is inadvertently selected, the volume defaults to loud. For now, I am pluging in headphones prior to going to sleep. Then if I do hit a video link by mistake, it cannot be heard. Should be a better way, but I haven't found it.
