SAR levels using 2 esims
Has Apple measured the SAR levels of an iPhone 13 Pro Max with an active gsm esim and an active CDMA esim operating simultaneously, along with active WiFi and Bluetooth?

There is no CDMA eSIM.
This is a user forum, you are not addressing apple here just fellow users
if it was me then I would inquire toward apple to get your anwsers
you can try contacting apple Support -> Get Support (apple.com)

Having two accounts doesn't matter as you can only use one line at a time since there are not two radios in your iPhone. So it is measured with the one (and only) radio.
The SAR levels have nothing to do with the SIM nor eSIM. They have to do with the physical radios in the iPhone sending and receiving signals to the network. Those don’t change just because you program dual eSIMs.
Already answered by Michael Black's excellent post and link above yours.
ok. I’ll put it another way. I have a T-Mobile and Verizon line active on my iPhone 13 Pro Max that were activated with 2 esims. Are the RF emissions greater because there are 2 radio connections active on my phone?

No. You don't. There is only one eSIM in the phone. One line is on the eSIM, the other is on a physical nano-SIM.
“On iPhone 13 Pro Max, iPhone 13 Pro, iPhone 13, and iPhone 13 mini, you can use two eSIMs for two or more cellular plans—two active at the same time—or a physical nano-SIM for a cellular plan and an eSIM for one or more other cellular plans.”
If the radios are sending and receiving signals from a T-Mobile and Verizon line in the same phone, are the SAR levels more than if there was only 1 active phone line? Did Apple measure the SAR levels of the iPhone 13 Pro Max with 2 active phone lines?
That isn't correct Tim: Using Dual SIM with an eSIM - Apple Support
Correct, which I already pointed out. But did you read the reply from Mr. Black?
