The whole process is described in Set up your iPhone, iPad or iPod touch
Make a backup from your old iPhone to iCloud.
Use Quick Start to transfer your settings to your new iPhone.
Let the App store download your purchased app to your new iPhone.
When it's finished you have nothing more to do other than sign in to (some) of your third-party services.
