What's your location? If you have access to a government QR code, you'll have an opportunity to add that to the Health app's vaccination records, and from there it can be added to Apple Wallet.
Other than that, the old fashioned way is to just take a photo of your vaccination card. I haven't been able to get my child's vaccination record on a government website and ended up just taking a photo of it and using that.
