This is almost certainly a scam. Apple doesn't prompt its users to fill out surveys for using their products. If this is a website, it probably changes based on your browser, for example if you saw this pop-up in Google Chrome, it would likely show up as a Chrome Visitor Survey 2021.
You should not click on or download anything from this survey/website.
