Suggestive ads and age rating

How are age ratings decided? My daughter took a screenshot of an ad for Adorable Home (12+ age rating) which shows a man watching a woman in her bra/underwear/bikini dancing for him on a stripper pole. The game itself has ads that ask the user if they want to “hook up” tonight and then suggests games with a 17+ age rating.

Will Apple change the age rating on the game? If yes, when? If no, why not?

Will Apple change their rules so that only age appropriate ads that correspond with the age rating are displayed? If yes, when? If no, why not?

Will Apple create a “report” mechanism in their App store? Or require one in their games? Currently there isn’t one. If yes, when? If no, why not?
