How do I restore the weather app that came on my iPhone phone 12 mini running iOS 15.3?

Go to the last screen which is the App Library add it back to your home screen.

Have you tried re-downloading it from the Apple App Store yet?

The Apple App Store app should be on the "main" screen on your iPhone.
