Heart rate notification while standing

I have POTS, which means my heart rate increases significantly when I stand. I want to be able to get a notification when my heart rate goes over 140 after standing, because this will alert me to sit down before I pass out. Is there a setting or an external app where I can do this?

(the high heart rate notification on the health app is for alerting when inactive, I just need a general warning every time it exceeds a certain number)

Best to consult with your medical provider about your particular Postural Orthostatic Tachycardia Syndrome (POTS) condition, and for best monitoring needs and treatment options—but I don't think Apple Watch notifications are appropriate here, as the anomalous rate tests are for periods of inactivity as you've noted, and as the sampling interval is not documented and may well be ten minutes and/or variable between pulse rate samples, and it seems quite possible for a syncopal episode to arise more quickly than that.
