How do I get a remote management username and password?

As per the following linked article, the organisation that installed MDM on your iPad can provide the username and password and/or remove MDM from the iPad:

You can also contact Apple Support for further guidance as follows:
- Online:--> Genius Bar Reservation and Apple Support Options - Apple;
- Contacting a local Apple Store or AASP among the list provided above but also again as follows Apple – Location Selector; or
- Using the "Apple Support" app, available and downloadable from App Store.
