I have a shortcut set up to open an app at a specific time during weekdays, but since updating to 15.3 the shortcut does not run. I get a notification that says "Tap to run..." but tapping the notification does nothing. iPhone SE (1st gen).

We understand you're experiencing an issue with a shortcut not running as expected.
Are you able to run other shortcuts normally?
If so, try removing this shortcut and creating it again.

Now if I add a "Wait" action before the "Open App" action and unlock my phone within the specific amount of wait time, it works. But this will only work if I unlock the phone within the specified amount of wait time and adding too long of a wait time would defeat the purpose. The desired workflow would be to either have the automation run after tapping the notification (being prompted to unlock the phone if locked) or after unlocking the phone for the 1st time after the shortcut notification appears.

This is the only shortcut I have. I have tried creating it again, and still no success. Tapping the notification that says "Tap to run" does nothing when I tap it either from the lock screen or if the phone is unlocked. I have tried it with "Ask Before Running" both enabled and disabled. It runs if the phone is unlocked. But if the phone is locked it won't run. I won't always have my phone open when I want this to run.
