Regarding documents required to claim to the insurance company.

On February 16, I repaired my iPhone at the Apple Store in Lincoln Road, Miami. However, at that time, I didn't know that I have an insurance and of course I said “No” when technician asked me do I have an insurance. But now I realize I have it and need some documents for insurance company I didn’t get on 2/16. So if it’s possible could you send it to me by email?
