Screen time passcode reset not working
I forgot my screen time passcode. I pressed forgot passcode and entered my apple it but it just adds another failed attempt when i try.

I'm not aware of anything beyond the Apple Support document directions for resolving the screen time pass code. I included a link to the document below, however, in the document is a reference point minimum 13.4 OS version. You mention iOS 12, I'm assuming that applies to IpadOS as well. So I'd first suggest upgrading your iPad to the latest version then follow the document.
If you have an iPad model that is no longer supported for versions iPad 15.x, I'd upgrade to that minimum 13.4 threshold. If you have an iPad that is no longer supported and it doesn't support above your current 12 version, I don't have any suggestions (maybe another poster will) other than purchasing a more recent iPad model.

Troubleshooting a Locked Device:
Hold it Right There!
Enter the passcode incorrectly too many times, and you'd likely be locked out for good. So, hold it right there! It would be the one you've always entered. If you do not know it, then look for it in a location you may have stored it in.
