When I try to make a phone call my iPhone 13 says "call failed. "I went into settings and looked at call forwarding which I do not use by the way, it did not have a toggle and was just twirling. I reset to factory settings, airplane mode is not on etc.

Calling is a phone carrier feature, contact yours to fix the issue.

This worked on my iPhone 6+ with IOS 12.5.5. I'd be surprised if it works on your 13 with IOS 15. But if so...something strange is going on out there.
Try: SETTINGS >> CELLULAR DATA (turn on if off) >> CELLULAR DATA OPTION >> ENABLE LTE to OFF

I did. The SIM works fine in another phone. Its bug where the iPhone is not reconnecting to the internal mike and speaker after using Bluetooth. My iPhone works fine with several Bluetooth earpieces but not make calls without them connected.
My iPhone 13th stopped allowing me outgoing calls possibly incoming also, it just says call failed
