Every time I save my passwords in my iPhone, it gets compromised in a data breach. Is saving my passwords in my phone making me more susceptible to data breach?

iPhone is alerting you to your passwords in data breaches from other sources. Passwords on iPhone are completely safe.

My larger question encompassing this is 1) how has this happened multiple times and 2) how does it happen across so many different platforms?
I appreciate that Apple alerts you to when your information has been compromised, but it just doesn’t make sense. Is data hacking simply more common than I think?
