Battery life seems to be unaffected, data did not come back on restart or anything, so at this point it's primarily cosmetic, but it is odd.

Troubleshooting Builtin Apps:
Two Thoughts on This:
A. Reset the Home Screen Layout:
If Builtin Apps are not working as they should, then you need to reset the Home Screen Layout...
Go Here: Move and Organize Apps on iPhone - Apple Support
You'd have to add back all the widgets that you can recall being back on there.
Scroll to: Reset the Home Screen and Apps to their Original Layout
or...
B. Set to Factory Settings:
If it comes down to is, you could restore your iPhone to Factory Settings. Important: If there are items on it (i.e. Photos), then enable those items in iCloud Settings on both the iPhone and on your Mac, so you can easily grab them. Important: Be certain to create a backup of your iPhone prior to resetting it. That way, you can have something to restore your iPhone from, should anything go wrong afterwards.
Did you recently restore the phone?
As in the thread a year ago there is no logical explanation,
The only time it happens to me is after DFU.
This has happened to me also, a few times. Seems like it’s not related to any updates, has happened since 14.6 on my iPhone XR. Everything appears to work as normal.
