activity app not appearing on home screen

Do you mean the Activity App? If so, download it from the App Store: Download apps and games on your iPhone or iPad - Apple Support. You need an Apple Watch for the App.

Not sure which iOS version you are using so I will have to assume that you may be using either iOS 14 or iOS 15.

Have you looked for the Activity app in the App Library? The App Library is the last Home Screen on your iPhone when you continue to SWIPE LEFT until you reach it. Once you are in the App Library, then you can search and find the Activity app. Once found, then you can DRAG the app out of there and place it on your favorite spot of the Home Screen.
