I've recently purchased a "brand new" iPhone from a 3rd party and can see some pen markings under the plastic wrap seal.
Is it safe to assume it's been opened and re-sealed?
I've also checked the IMEI on the box online and am getting Past First Activation: Yes. What does that mean?

It is not "brand new".
If it were "brand new" you would have been purchasing it from Apple or an authorized retailer, not a "3rd party".
IMEI checkers are pretty much worthless. They don't have legitimate access to that information.
The screen shot finally appeared. That means your used iPhone was originally sold on September 13, 2021, you will have to pay for telephone support and it is too late to purchase AppleCare + for it.
Any phone from a 3rd party most likely is not “new”. If you have the serial number, enter it here and see what it says: Check Your Service and Support Coverage - Apple Support
The iPhone 13 does not come with plastic wrap so it was definitely resealed. The box should have looked like the one in this article: https://9to5mac.com/2021/09/17/iphone-13-box/
This is what I get:
