How do I restore the iCloud icon on my iPhone?

There hasn't been an iCloud App for quite a while. Do you mean a third-party App? If so, Redownload apps and games from Apple - Apple Support.
If you moved it to the App Library, go to the App Library (swipe to the last page on your Home Screen), and long press on the App's icon. You can then move it back to the Home Screen.
