New files are not registering on devices or iCloud.

Recent files do not show up on devices or in iCloud, but I can find them with Finder or Spotlight, so they are not entirely gone. How can I get them to register in the usual places again?! Where do they go, such that they no longer show up on desktop or documents anywhere?
