this is extremely annoying.
I do not adjust any Safari settings, running it in default. It happens routinely on any website.

I am having this issue too. Since the upgrade, Safari has been behaving oddly. The biggest thing is that the back button will just often disappear. If I click through a few links, it is usually still there and then randomly, I click another and it is gone.
Also a new odd behavior is that when I click on a link sometimes it starts to open a new page, but then goes right back to the one I was on. But then if I click the same link a again, it navigates to the URL I selected.
This is all very strange and highly annoying and disruptive.
I have restarted my ipad and the safari app many times, cleared cookies, cleared history, tried different settings, etc. Nothing seems to work.

This has also started happening to me! I updated to 15.4.1 a few days ago, and as soon as I did, Safari started behaving exactly as yours is!
I would click on a link, it would start loading, then take me back to the page I was just on. And it would remove my navigation history so I couldn’t click on the back button!
No amount of restarting the ipad, or safari has helped so far.
It’s getting fairly annoying as I use the back button fairly frequently, and having it just stop working is extremely annoying.

Next, let's clear the history and close Safari. Reopen and test:
Clear the history and cookies from Safari on your iPhone, iPad, or iPod touch
Close an app on your iPhone or iPod touch
If the issue persists, Contact Apple for support and service

Same here, to a point I am considering dropping Safari for Chrome ! From what I can see, the issue is caused by some sort of automatic removing of the history. Very odd.

Are you using any accessories with your iPad, such as a keyboard, when this is happening? Have you restarted your iPad since this began happening?

Restarted it many times, but thanks. Also not using any accessories at all.
I've already done this a couple of times to no avail. So thats not it either.

Try updating to iPadOS 15.4.1 - the most recent version.
I am using iPadOS 15.4.1. That is when the issues started as far as I recall. It was very recent.
