If my son already has an ID set up under our Family Sharing plan, how do I add his née iPhone?

You need to do it from this Apple Article -> Check your Apple ID device list to see where you're signed in

May be you need to do it from his account, bacause he has his own Apple ID.

Add the new device & remove the old one.
So there is not a way to have two devices under same ID? If not, do I set up a 2nd ID for him so that both his iPhone and iPad are listed?

That is possible.
I had obviously misunderstood you, about old and new iPhone. Not knowing he wants to keep both devices.
Just add the new device to the list.
How do I add the device the the Danny R. ID. It does not give me an option to do so.
