Do you have to add more storage by purchasing it, when you have 5GB and your phone says you do not have enough storage to back it up?

When you are notified that there is not enough storage to backup your iPhone, then that means that there is not enough space on iCloud to backup your iPhone there.
The 5GB of available space that you may be looking at is on your iPhone. Your iPhone is not backed up on your iPhone so that 5GB of available space is irrelevant.
You can purchase more space on iCloud so that your iPhone can be backed up to it, however, this will not increase the overal available space on your iPhone. The total storage space capacity on your iPhone will always remain the same.

If you want to backup on iCloud then yes you need to buy more iCloud storage. Else you may take a backup on a PC/Mac.
