Why does my iPhone 13 say I have an Apple Watch? I’ve never had one but it says it’s an option to repair it. I’m tempted to pair it so I can see what it is, but I worry about privacy issues.

There is nothing in that screenshot that would indicate to me that the phone thinks it has been paired to an Apple Watch. That's just boiler plate.
