Once a family member is added, how do I create a separate profile for that family member?

So my sister has her own iPad but we share the television set. Does that mean she enters her Apple ID number and that allows her to have a separate account but still using family sharing?
Yes. Family Sharing perks are available to her on any device when signed in with her account.
And does it mean that we can then switch off on the television set, depending who is watching?
Yes. Users may switch between any of the defined users.
For the Apple TV box (4th generation or later): Set up multiple users on Apple TV - Apple Support and Switch to another user - Use tvOS Control Center on Apple TV - Apple Support
For third party devices with the Apple TV app: Add additional Apple ID accounts - Family Sharing in the Apple TV app - Apple Support
That's not actually helpful because it doesn't help with "Up Next," which allows people to keep track of what they watch individually --- which doesn't matter unless people are watching the same series, or stopped a film in the middle of watching, and a different family member wants to watch the same movie. And nobody can create their own actual Apple TV list for their own watching.
So setting up profiles for a single device is kind of pointless if you can't set it up throughout the system. It only works if you're using a single piece of hardware (and my TV has 4K capability separate from my Apple TV device).
For devices that allow for multiple user accounts (Apple TV box, third party devices with the Apple TV app),
switching the active user would also switch the Up Next settings to that user’s preferences.

There are no “profiles” as different from the Apple ID accounts. With Family Sharing, each user in the group accesses shared content and services on their own Apple ID account.
That totally ***** because if two people want to watch the same program on "Up Next," we can't keep track of what we are watching individually. **** you Apple.
I bought Paramount Plus through Apple. I am cancelling that and buying it separately and then adding the family member as a profile. So Apple loses out.
