PUK code needed, after restoring iPad to factory settings

I have been hacked and am restoring all my devices, There is a phone number attached to my iPad that is not mine, called apple support and they walked me through restoring my iPad to factory settings, but now it is asking me for a PUK key, I am able to haver a Verizon number connected to my iPad but I never activated it, I fear whoever has access to the iPad has it connected, what do I do?

Model is 2013 iPad Air with Wifi and Cellular.
