Immunizations not displayed in Health app

I was a version behind in iOS. One I installed the most recent version I was able to add my immunization record.

Here is a screenshot from his phone (second screenshot). He has Virginia Dept of Health as an option on his phone. Virginia Department of Health isn’t an option on my phone (first screenshot).
